Whether you or a loved one has just been diagnosed with a life limiting disease, dealing with end of life issues or trying to cope with the loss of a loved one, our team is here to help. We also work closely with other community partners to assist you with your needs in the comfort of your own home.
Community Outreach Program can assist you with:
- A consultation with one of the Hospice Palliative Physicians can be arranged in partnership with your family doctor to discuss pain and symptom issues.
- Individual needs assessment for supportive, spiritual and advanced care as well as end of life planning.
- One on one consultation with Supportive Care Coordinator to meet spiritual and psycho-social needs.
- Grief and bereavement support for individuals coping with a diagnosis of a life limiting illness as well as support of those coping with the loss of a loved one.
Our day programs provide a caring environment where individuals living with a life limiting illness can interact with others experiencing similar challenges and gain support from a dedicated team of staff and volunteers.
Individual participation in the Day Wellness Program promotes sharing and provides an opportunity to re-engage in activities that bring meaning and value to daily living. It also provides a day of respite to families caring for their loved one.
