Applying to move into long-term care is a straight-forward 4 step process!

We work together with our Local Health Integrated Network for Hamilton Niagara Haldimand Brant to ensure you or your loved one find the home and care you need at St. Joseph’s Villa.

  1. Contact the (HNHB – LHIN) Hamilton Niagara Haldimand Brant Local Health Integrated Network and request to be registered for placement at St. Joseph’s Villa. HNHB – LHIN will arrange for a case manager to visit and assess your medical care needs.Phone: 905-523-8600
  2. Once you or your loved one is deemed eligible for long-term care, HNHB – LHIN will give you a list of long-term care homes to choose from and rank according to your preferences.
  3. Since you have included St. Joseph’s Villa in your choices, HNHB-LHIN will send us a copy of the medical assessment The assessment will be taken to the Villa’s Admission Committee for approval.
  4. Once a suitable bed becomes available, the HNHB – LHIN will contact you and you will have a brief time to make your final decision!

It is important to inform and update your case manager of any contributing health, cultural, religious or environmental conditions and medical and health changes along the way.
