The Behavioral Supports Ontario (BSO) project is a provincial initiative focusing on providing quality care for individuals with complex and challenging mental health, dementia or other neurological conditions, in an environment that is based on safety, high quality, evidence-based care and practice.

We are expanding our service to include TRANSITIONAL SUPPORT to seniors in the community transitioning to Long Term Care. The TRANSITIONAL LEAD will work in collaboration with client, family, and community partners to facilitate a holistic and supportive approach to care for persons presenting as high risk for responsive behaviors on transition to a long term care facilities. Six transitional leaders work in different areas of the LHHB LHIN.

Successful applicants for these permanent full time positions must possess:

  • University degree and/or current registration in a regulated Health Care Profession (RN,PT,OT or RSW)
  • A minimum of five years health care experience working with complex older adults with mental health illness
  • Demonstrated well developed knowledge of psychiatric and geriatric disorders
  • Proven experience working as a leader, mentor, preceptor, educator and problem solver with Health Care Professions and families
  • Extensive working knowledge of community resources for Geriatrics/Mental Health
  • Proficiency in computer and software use such as Microsoft,, PCC, Clinical connect
  • Ability to adapt professionally and efficiently to changing conditions and priorities
