Support for individuals and families in a home like setting
Our hospice care services provide care for the body, mind and soul with a multi-disciplinary team who journey with the resident and their family through their final days. Supporting the individual, their family and friends in this home like setting is our privilege and priority.
Care is available 24/7 for individuals who are facing a life-limiting illness who have difficulty coping at home, who do not have caregiver support or for those who choose not to die at home. Margaret’s Place Hospice care may be the welcomed alternative. Registered Nurses, Registered Practical Nurses and Personal Support Workers are on site to provide the care that is required which is supported by physicians.
If an individual or their Substitute Decision Maker (SDM) expresses interest in hospice care as an option for their end-of-life care needs, a referral to the Home & Community Care Support Services (HCCSS) is required.
This will initiate the process to determine eligibility for hospice care. The referral to HCCSS can be made by anyone – family member, physicians, support worker. HCCSS will guide you through the process. There will be an option to request Margaret’s Place Hospice as the hospice of choice. When all the necessary processes are complete, HCCSS will then contact Margaret’s Place to advise who wishes to receive end-of-life care at the hospice.
